Job summary
Continuum Hospice & Palliative Care Massachusetts is seeking Per Diem Hospice Nurse Practitioners to deliver compassionate patient care in the Monterey, Santa Cruz and Santa Clara County area. You will provide palliation, assess health histories, and develop personalized care plans under physician supervision.
You will collaborate with the hospice interdisciplinary team, educate patients and families, and ensure compliance with regulatory requirements while supporting a respectful, dignified
Qualifications
- Graduation from an accredited School of Nursing.
- Master's degree with NP certification and one year hospice/palliative care experience.
- Current nursing license in CA and CPR certification.
Responsibilities
- Deliver comprehensive medical care and palliate for terminal illnesses under physician supervision.
- Collaborate with hospice team to assess eligibility and plan ongoing care.
- Coordinate patient care with the interdisciplinary team for a holistic approach.
- Educate patients and families on care options, goals, and expectations.
- Recommend initial certification and recertification for hospice eligibility.
- Stay compliant with Medicare/Medicaid and state/federal hospice rules.
