Job summary
Continuum Hospice is actively hiring Per Diem dedicated Hospice Nurse Practitioners to deliver compassionate patient care in Monterey, Santa Cruz and Santa Clara County. You will evaluate health histories, diagnose conditions, and create personalized care plans in collaboration with patients and families.
The role emphasizes comprehensive hospice care, adherence to the Nurse Practice Act, and ongoing education to support patients at end of life.
Qualifications
- Master's degree in Nursing and NP program accreditation.
- Current RN license with CPR certification.
- California NP regulatory standards met.
- 1+ year NP experience in hospice/palliative care recommended.
- Strong communication skills (verbal & written).
- Computer proficiency and EMR experience required.
Responsibilities
- Provide comprehensive hospice medical care under physician supervision.
- Collaborate with hospice team to assess eligibility and plan care.
- Coordinate patient care with interdisciplinary team and families.
- Educate patients and families on care goals and options.
- Support certification/recertification decisions with Medical Director.
- Stay compliant with Medicare/Medicaid and state/federal rules.
