Job summary

Continuum Hospice is hiring Per Diem dedicated Hospice Nurse Practitioners to deliver comprehensive hospice care in and around Monterey, Santa Cruz and Santa Clara County. You will evaluate health histories, diagnose conditions, develop personalized care plans, and collaborate with patients, families, and the care team to provide compassionate, holistic care.

Requirements include an accredited nursing degree, NP program completion, California RN license with CPR, and NP experience in hospice or

Qualifications

• Graduation from an accredited School of Nursing.
• Completion of an accredited Nurse Practitioner Program with board standards or equivalent.
• Current nursing license in California and CPR certification.
• Master's degree with NP experience and hospice/palliative care exposure.
• Certifications from approved bodies (e.g., CHPN) preferred.
• Excellent verbal/written communication; computer proficiency including EMR.
• Valid driver's license and reliable transportation.

Responsibilities

• Deliver compassionate medical care for hospice patients under physician supervision.
• Collaborate with attending physician and hospice team on care plans.
• Coordinate and oversee patient care with interdisciplinary team.
• Educate patients and families, addressing medical, psychological, and spiritual needs.
• Provide input on initial and recertification eligibility for hospice care.
• Maintain up-to-date knowledge of Medicare/Medicaid and state/federal rules.
