Job summary
Visiting Nurse Health System is seeking a compassionate Nurse Practitioner to join their Hospice/Palliative team serving Henry County, SW Atlanta, Clayton, Cobb, and Cherokee counties. You will collaborate with physicians to deliver end-of-life and palliative care, and educate healthcare professionals in the field.
The role requires GA Advanced Practice Nursing licensure, NP certification (gerontology/family/adult), CPR, and at least two years managing Medicare patients; prior home-based care
Qualifications
- Must hold a current Georgia license for Advanced Practice Nursing.
- Certified gerontologic, family, or adult Nurse Practitioner by a recognized board.
- Two years of Medicare patient management experience.
- CPR certification.
- Master's degree in Nursing from an accredited program.
Responsibilities
- Deliver end-of-life and palliative care with a physician in an active practice.
- Act as a consultant and educator for hospice programs and healthcare professionals.
- Bridge hospitals and hospices to improve care for terminally ill patients.
