Qualifications

Current unrestricted Nurse Practitioner in Michigan required.
One year of Medicare-certified home health/hospice experience preferred.
Knowledge of home health/hospice and outpatient services preferred.
Ability to support Hospice Medical Director/designee and communicate improvements.
Excellent interpersonal skills, judgement, organization, confidentiality and collaboration.

Responsibilities

Conduct routine and urgent assessments in patient homes; available PRN/part-time.
Perform medical evaluations of elderly, frail patients in home settings.
Order and interpret tests; prescribe medications as licensed.
Coordinate care with palliative team and primary physicians.
Maintain accurate patient records and support reimbursement.
