Job summary

AdvisaCare Hospice in Michigan is seeking a full-time Hospice RN to deliver compassionate, one-on-one care in patients' homes, focusing on dignity and comfort.

You will perform initial and ongoing assessments, implement the care plan, educate families, supervise paraprofessionals, and participate in IDG conferences to drive quality and satisfaction. A valid Michigan RN license and hospice experience are required.

Qualifications

• Current Michigan license in good standing.
• Hospice experience preferred.
• Ability to provide emotional support and educate families.

Responsibilities

• Initial and ongoing assessment of patient needs.
• Implement the individualized plan of care and revisions as needed.
• Compliance with policies, procedures, and Medicare rules.
• Educate patient and family on disease and end-of-life care.
• Supervise paraprofessionals providing services to patient.
• Prepare clinical notes and participate in IDG conferences.
• Contribute to performance improvement initiatives.
• Maintain high levels of satisfaction with clients and families.
• Communicate information using existing processes and technology.
