Responsibilities

• Conduct accurate history and physical exams for hospice eligibility and symptom management
• Coordinate with care teams to develop and implement goals of care
• Document encounters and orders in the EMR promptly and accurately
• Provide education and support to patients, families, and staff
• Participate in on-call coverage and quality assurance activities
