Job summary

AdvisaCare Hospice and Palliative Care Program in Grand Rapids, MI is seeking a part-time Nurse Practitioner to conduct home-based assessments and manage advanced illness patients.

You will order tests, coordinate with physicians and the interdisciplinary team, and participate in quality initiatives. The role requires strong clinical skills, excellent communication, and the ability to work independently in patients' homes.

Qualifications

• Current unrestricted Nurse Practitioner license in Michigan is required.
• Experience in Medicare-certified home health/hospice settings is preferred.
• Strong communication and teamwork skills with attention to confidentiality.

Responsibilities

• Perform routine and urgent assessments in patients' homes.
• Order and interpret tests within scope and coordinate care with the Medical Director/designee.
• Prescribe medications as delegated and document care within 24 hours of visits.
• Coordinate interdisciplinary team involvement and ensure continuity of care.
• Support patient and family goals of care and Advance Care Planning.
