Qualifications

• Graduate from an accredited school of nursing and accredited Nurse Practitioner program.
• Master’s degree with a specialty certification preferred.
• Accreditation from an approved certifying body for advanced practice nursing as required by the state.
• Current nursing license in the state of Michigan.
• Minimum of one year experience in hospice or palliative care.

Responsibilities

• Provide medical care and manage terminal illness under supervision of a collaborating physician.
• Perform physical examinations and order/interpret tests and document findings.
• Develop individualized plans of care and coordinate hospice services.
• Complete hospice face-to-face visit requirements for eligibility.
• Communicate plan of care with Chief Medical Officer and team.
• Educate patients and families on care and symptom management in collaboration with the team.
• Maintain accurate medical records and documentation.
• Coordinate with interdisciplinary group to ensure quality patient care.
