Job summary

VIA Health Partners is seeking a licensed Nurse Practitioner to provide comprehensive end-of-life care within our hospice program. You will collaborate with physicians, nurses, social workers, and other clinicians to deliver compassionate care to patients and families in our community.

The role focuses on managing pain and symptoms, guiding prognosis discussions, and coordinating treatment plans in alignment with patient goals.

Qualifications

• Minimum 1 year related experience.
• Experience in direct care of patients with life limiting illnesses required.

Responsibilities

• Promote high quality end-of-life care in hospice setting.
• Diagnose, treat and manage pain and symptoms at end of life.
• Counsel patients and families on disease progression, prognosis and treatment options.
• Prescribe and coordinate therapies and medications within ethical medical practice.
• Participate in team discussions to improve patient care and outcomes.
