Job summary

VIA Health Partners in Charlotte, NC is seeking a compassionate Certified Nursing Assistant to deliver essential patient care in our community-based hospice. You will assist with activities of daily living, vital signs, mobility, and comfort measures under RN supervision.

Qualifications

• Certified Nursing Assistant with current NC DHHS listing and CPR certification.
• High School diploma preferred; related field training or certification required.
• Experience in home care or geriatrics is a plus.

Responsibilities

• Delivery of patient care including ADL assistance and safety measures.
• Maintain infection control, assist with transfers, vital signs, and comfort.
• Documentation of care following HPCCR guidelines and team processes.
