Job summary

Beaumont ASHN in Indianapolis seeks a Registered Nurse (RN) Hospice General Inpatient Nurse Case Manager to provide end-of-life care in an inpatient hospice setting. You will apply holistic, family-centered care and coordinate services to support comfort and quality of life.

The role requires 3+ years acute care experience, an Indiana RN license, and CPR. You will document care, support families, and participate in discharge planning and QAPI activities as part of ASHN standards.

Qualifications

• RN license in the state where agency is located.
• Bachelor's degree in nursing preferred.
• CPR certification required or preferred.
• Strong communication and documentation skills.

Responsibilities

• Plan and deliver care using the nursing process (assessment, planning, interventions, implementation, evaluation).
• Interact with patients, families and interdisciplinary team with professionalism.
• Maintain patient dignity and respect cultural beliefs at end of life.
• Provide family/caregiver support at end-of-life and during transitions.
• Attend interdisciplinary team meetings and participate in discharge planning.
• Maintain complete clinical documentation per agency policy and Medicare guidelines.
