Redcrest Hospice LLC in Indianapolis, IN, is seeking an experienced RN to provide end-of-life care in an inpatient hospice setting, delivering compassionate, family-centered support and coordinating care with the health system team.

Requirements include active Indiana RN license, minimum 3 years acute care, hospice experience preferred, CPR certification, and strong written/verbal communication, critical thinking, and organizational skills.

• CPR Certified
• Strong written and verbal communication skills
• Critical-thinking skills needed to assess a patient’s situation and act as necessary
• Organizational skills with the ability to prioritize in a rapidly changing day-to-day environment

Responsabilités

• Plan and deliver care to patients using the nursing process (assessment, planning, interventions, implementation, evaluation).
• Interact with patients, families and interdisciplinary team while maintaining professional nursing standards.
• Maintain dignity of the dying patient and respect spiritual and cultural beliefs.
• Provide support to families at end-of-life and at the time of death.
• Attend interdisciplinary team meetings for care discussions and discharge planning.
• Maintain documentation for continuity of care (direct care, medications, calls, visits).
• Participate in QAPI activities as requested.
• Coordinate patient care for transitions from hospital to hospice setting.
• Complete all clinical documentation per agency policy and Medicare guidelines.
• Attend all mandatory meetings.
• Other duties as assigned.
