Job summary

Brighton Hospice in Indianapolis seeks a Registered Nurse Case Manager to plan, organize and direct hospice care in patients' homes and communities. You will conduct comprehensive assessments, develop care plans and coordinate with a multidisciplinary team to deliver compassionate end-of-life care.

The role requires an active RN license in good standing, ability to travel within the service area, strong teamwork, time management, and experience with electronic medical records.

Qualifications

• Active RN license in good standing in the state(s) of practice.
• Ability to collaborate with a multi-disciplinary team including nurses, social workers, chaplains, volunteers, and CNA leadership.
• Capable of working independently with phone, email, and in-person communications.
• Strong time-management to schedule visits, document care, and attend meetings.
• Willingness and ability to drive within the service area with reliable transportation.

Responsibilities

• Completes initial, comprehensive, and ongoing assessments to determine hospice needs.
• Develops and updates plans of care with nursing diagnoses and palliative actions.
• Coordinates patient care for assigned caseload and Revises plans as patient needs change.
