Job summary

Alternate Solutions Health Network in Indianapolis is seeking an experienced Registered Nurse (RN) for Hospice, General Inpatient Nurse-Case Manager. The role focuses on end-of-life care in a holistic, family-centered manner and includes coordinating patient care across the health system to ensure smooth transitions.

The position requires strong communication and critical-thinking skills, with a minimum of 3 years acute care experience; hospice experience is preferred.

Qualifications

• RN with Diploma or ADN; Bachelor’s in Nursing preferred.
• Active RN license in IN; no board actions.
• CPR certification required.

Responsibilities

• Plan and deliver care using the nursing process (assessment, planning, interventions, implementation, evaluation).
• Interact with patients, families and interdisciplinary team while maintaining nursing standards.
• Maintain patient dignity and support spiritual/cultural beliefs at end of life.
• Provide support to families/caregivers at end of life and at time of death.
• Attend interdisciplinary team meetings for care discussions and plan of care.
• Maintain documentation for continuity of care including meds, visits and communications.
• Participate in QAPI activities as requested.
• Coordinate care for transitions from hospital to hospice settings.
