Job summary

Apploi is hiring a Registered Nurse for hospice care in Indianapolis. The role focuses on planning, organizing, and directing patient care in home settings, with emphasis on community health education and family engagement.

You will assess patient needs, develop and revise plans of care, coordinate with the interdisciplinary team, and ensure accurate documentation while supporting patients and families through end-of-life care.

Qualifications

• Graduate of an accredited school of nursing.
• Licensed RN in good standing to practice.
• One year of professional nurse experience within the last three years; hospice/public health/home care preferred.
• Ability to lift and transfer patients; valid driver’s license and reliable vehicle.
• CPR certification required.
• Excellent communication and problem-solving skills.

Responsibilities

• Complete initial and ongoing patient assessments to determine hospice needs.
• Provide professional nursing care per the plan of care.
• Develop and revise care plans with patient and family involvement.
• Coordinate care with the interdisciplinary team.
• Maintain accurate clinical notes and reports.
• Participate in on-call duties and ensure equipment arrangements.
