Central Coast VNA & Hospice is seeking an RN Hospice Case Manager to coordinate and deliver skilled nursing care and support services in patients’ homes across the Central Coast. You’ll collaborate with physicians and the care team to ensure comfort, safety, and quality of life for patients and families.
The role requires California RN licensure and at least one year of Home Health or Hospice experience, with CPR certification and strong teamwork abilities.

The Hospice Case Manager collaborates with physicians and the interdisciplinary care team to identify, coordinate, and address the home care needs of patients, families, and caregivers. The Hospice Case Manager provides skilled nursing care and supportive services using the nursing process, with a focus on patient comfort, safety, and qualify of life.
They refers and coordinates other disciplines and specialty services and facilitates transitions to other levels of care or services, as needed. Manages patient care in accordance with agency standards, applicable regulations, and payer requirements. Provides education and support to patients, families, and caregivers and prepares patients and families for discharge when appropriate.

Monday - Friday w/ rotating weekends

Compensation is influenced by labor market conditions and various factors, including but not limited to: prevailing minimum wage in the area, relevant education, experience, skills, geographic location, performance, travel requirements, sales or revenue targets, and business needs.
