Job summary

Advocate Aurora Hospice in Green Bay is hiring a Home Health Aide to support hospice patients in the Green Bay market with occasional travel to nearby areas. The current schedule is Monday–Friday, 8:00 a.m.–4:30 p.m., with a 40-hour workweek and full benefits eligibility.

No prior experience required; certification as a nurse aide in the state is needed, along with BLS within 6 months and a valid driver's license. Training and HIPAA compliance are emphasized.

Qualifications

• Must be certified as a nurse aide in the state of practice.
• BLS for Healthcare Providers certification required within 6 months.
• Valid driver's license required.
• Complete all state and agency training and HIPAA privacy rules.

Responsibilities

• Provide competent patient care as assigned by RN and plan of care.
• Assist with personal care, feeding, and activities of daily living.
• Take and record vitals as directed and monitor patient condition.
• Document care provided and communicate changes to RN/manager.
• Maintain confidentiality and comply with HIPAA/privacy rules.
• Support scheduling, CQI activities, and team goals.
