Job summary

Choice Hospice, Inc. is seeking a Hospice Field Registered Nurse (RN) to join the Tyler, TX team.

This full-time role provides compassionate, patient-centered care in home and community settings, coordinates with physicians, social workers, and chaplains, and ensures comfort, dignity, and quality of life for patients and their families. You will perform initial and ongoing assessments, develop and adjust plans of care, educate families about disease progression and end-of-life care, and

Qualifications

• Current RN license in good standing.
• CPR certification required.
• Valid driver’s license and auto insurance.
• Hospice RN experience 2+ years preferred.
• HomeCare HomeBase experience preferred.

Responsibilities

• Initial and ongoing comprehensive assessments of patient needs.
• Develop, implement, and update plan of care.
• Educate patient and family on disease, end-of-life care, nutrition and self-care.
• Coordinate with IDG and provide staff training as needed.
• Determine services based on acuity and caregiver capabilities.
• Provide clinical directions to Hospice Aide and LPN/LVN.
• Document progress and communicate changes to physicians.
• Fulfill on-call responsibilities as assigned.
