Choice Hospice, Inc. is seeking an experienced Hospice Field LPN PRN in Safford, AZ. The role focuses on symptom management, comfort, and quality of life for patients with life-limiting illnesses in home or care settings.

Under RN direction, you will assess needs, plan and implement care, apply infection control, and maintain clear documentation while supporting families and the care team.

Formación

• Licensed LPN/LVN in Arizona is required.
• Ability to provide compassionate hospice care at home or in facilities.
• Strong communication and documentation skills.

Responsabilidades

• Identify patient needs under RN guidance using charts and team notes.
• Plan and implement care in collaboration with the RN per the POC.
• Apply infection control and standard precautions during care activities.
• Ensure continuity of care through quality patient care delivery.
• Communicate effectively with patient, family, and health professionals.
• Document outcomes of interventions with progress notes.
