Job summary

Qualifications

• Direct patient care experience preferred

Responsibilities

• Provide direct patient care to patients under the RN and according to the Aid Plan of Care
• Assist patients with safe transfers and ambulation per agency policy
• Report changes and document pertinent information for continuity of care
• Maintain infection control standards and provide a clean, safe home environment
• Communicate respectfully with patients, families, and staff
• Assist with nutrition and hydration as directed by the care plan
