Job summary

Affinity Health Management in Roanoke, VA is seeking a Certified Nursing Assistant to provide compassionate hospice care under RN supervision. You will perform personal care, monitor patient condition, and document observations in the EMR according to the plan of care.

The role emphasizes safety, confidentiality, and respectful communication with patients and families. Qualifications include VA CNA certification, strong communication skills, and prior hospice or home health experience preferred.

Qualifications

• Must hold VA CNA certification and current license.
• Ability to read and follow written instructions.
• Experience in hospice or home health care preferred.

Responsibilities

• Perform personal care after given assignment and attend to requests promptly, reporting to Case Manager if unable to perform a task.
• Provide patient and family with respectful communication while maintaining confidentiality.
• Ensure patient safety and use equipment properly (foot stools, side rails, O2, etc.).
• Assist with bathing, grooming, oral hygiene, shampoos and changing bed linens as assigned.
• Assist in dressing and undressing patients as needed.
• Assist in feeding as assigned; avoid home-made foods.
• Monitor skin care to prevent tissue breakdown over bony prominences.
• Report pain or changes in condition to RN Case Manager; be aware of caregiver dynamics.
• Assist with ambulation and prescribed exercises.
• Provide ROM and transfers/positioning as directed.
• Assist with medications reminders where allowed by state law.
• Assist with bedpans, urinals, and light housekeeping tasks.
• Perform ROM and simple procedures as ordered with competency.
• Provide respite for patients/families when on-site, as appropriate.
• Keep living area clean and orderly per procedures.
• Follow documentation and care standards; participate in quality improvement activities.
• Respect patient and family environment and needs.
• As a mandatory reporter, report non-compliance to VA Dept. of Health within 14 days.
