Job summary

Affinity Health Management in Virginia is seeking a hospice aide to work under RN supervision, performing personal care, documenting observations, and promoting patient comfort as part of the plan of care. The role emphasizes safety, confidentiality, and quality of care.

Qualifications

• Completion of a state approved certified nursing assistant course or equivalent training with current license.
• High school diploma or General Equivalency Diploma (GED).
• Ability to read and follow written instructions and document care given.
• Understands hospice philosophy and comfortable providing specialized care to the terminally ill.
• Prior experience in hospice, orderly, acute, rehabilitative, or long term care facility; 1 year preferred.
• Basic computer skills to document in EMR.
• At least 18 years of age.
• Licensed driver with insured vehicle in good working order.
• Fully vaccinated.

Responsibilities

• Perform personal care tasks after patient assignment and respond promptly to requests.
• Provide patient and family with compassionate communication while maintaining confidentiality.
• Ensure safety of patient and proper use of equipment.
• Provide baths, back rubs, oral hygiene, shampoos, and bed linen changes.
• Assist in dressing and undressing the patient as assigned.
• Assist in feeding, avoiding homemade foods prepared by the aide.
• Monitor skin and prevent breakdown over bony prominences.
• Report pain or changes in patient conditions to the RN Case Manager.
• Assist in ambulation and prescribed exercises.
• Provide ROM and transfers/positioning as directed.
• Assist with medications reminders where allowed by state regulations.
• Assist with bedpans, urinals, light laundry and housekeeping tasks.
• Perform ROM and simple procedures as competency allows.
• Provide respite for patient/family/caregiver when on-site.
• Keep living area clean and orderly.
