Job summary

Anthem Hospice is seeking a PRN Certified Nursing Assistant in Glendale, AZ, to provide compassionate personal care to terminally ill patients under the direction of the nurse or nurse supervisor. Responsibilities include maintaining a safe home environment, assisting with meals, feeding, and activities of daily living, and supporting caregivers.

The role requires travel as needed, with a focus on maintaining patient independence and quality of life within an interdisciplinary care plan.

Qualifications

• High School graduate or equivalent preferred.
• Proof of state certification, if applicable.
• One-year direct patient care experience in an institutional setting.

Responsibilities

• Provides personal care under supervision of Nurse or Nurse Supervisor.
• Keeps patient in a safe and healthy environment and reports changes to the nurse.
• Assists with meals, feeding, and measuring/ preparing special diets as ordered by physician.
• Assists with ambulation and use of walker or wheelchair as taught by nurse.
• Assists with routine treatments and monitoring as taught by clinician.
