Job summary

Anthem Hospice Care is seeking a compassionate Certified Nursing Assistant to provide in-home personal care to terminally ill patients under nursing supervision. Responsibilities include assisting with hygiene, feeding, mobility, and routine activities, while promoting safety and comfort in the home environment.

The role involves documenting care, supporting family caregivers, and collaborating with the interdisciplinary team to execute the care plan.

Qualifications

• High School graduate or equivalent preferred.
• Proof of state certification, if applicable.
• One-year direct patient care experience in an institutional setting.

Responsibilities

• Provide personal care as outlined in the written assignment (hygiene, grooming, feeding, nutrition).
• Assist patient in ambulation and use of walker or wheelchair as directed by nurse.
• Support prescribed range-of-motion exercises under nurse/physical therapist supervision.
• Promote safety and a healthy home environment and support infection control per OSHA standards.
• Report changes in patient condition to the Renaissance Sunrise/Nurse and interdisciplinary team.
• Assist caregiver through basic care techniques and education.
• Maintain accurate documentation of care activities and care plan progress.

Education

High School diploma or equivalent

State certification (as applicable)

1 year direct patient care experience in an institutional setting
