Desert Valley Hospice and Palliative Care in Phoenix, AZ is seeking a compassionate Hospice Aide to support terminally ill patients and their families as part of the interdisciplinary team, following RN-directed plans of care.

Responsibilities include assisting with personal care, transfers, mobility, nourishment, and documenting observations; communicating changes to the hospice nurse; and maintaining patient rights and a professional appearance.

Competenze

• Must read, write, and speak English fluently.
• Completion of an approved training/competency program per law.
• ADN/certification or licensure as applicable; current status in good standing.
• Background checks and health clearance as required.

Mansioni

• Assist patients with personal hygiene, feeding, mobility and medication reminders.
• Provide care aligned with written instructions and RN plans.
• Support caregivers and reinforce teaching from the team.
• Document care provided and patient responses per policy.
• Communicate changes to the hospice nurse and interdisciplinary team.
• Participate in care planning and maintain professional conduct.
