Job summary

Desert Valley Hospice & Palliative Care in Phoenix seeks a dedicated Hospice Aide to provide compassionate care to terminally ill patients and support to families under the direction of the RN and interdisciplinary team.

You will assist with personal care, hygiene, mobility, nutrition, and medications reminders, document care, communicate changes to the nurse, participate in care planning, and maintain professionalism in a sensitive home and facility setting.

Qualifications

• CNA or equivalent training or competency program required.
• CPR, TB, and health clearance are required.
• Must meet state/federal nurse aide requirements and be listed in good standing.

Responsibilities

• Assist patients with personal hygiene, elimination, nutrition, ambulation, and medication reminders.
• Provide care per RN instructions and document care provided.
• Support caregivers and reinforce teaching by the hospice team.
• Communicate patient status and changes to the hospice nurse.
• Participate in development and implementation of the plan of care.
• Maintain professional appearance and confidentiality.
