Job summary

Affinity Health Management is seeking a Hospice Aide to work under the supervision of a registered nurse, performing tasks per the patient’s care plan to promote comfort and meet personal needs. The role emphasizes reporting observations and maintaining safety and continuity of care.

The aide will follow strict documentation practices, assist with activities of daily living, and support families while ensuring ethical, respectful, and compassionate care in a clinical setting.

Qualifications

• Completion of a state approved certified nursing assistant course or equivalent training.
• Nursing Assistant Certified (NAC) certification.
• Current Healthcare Provider CPR certification (hands-on verified).
• Current Food Handlers Permit.
• High school diploma or GED.
• Ability to read and follow written instructions.
• Must be 18 years of age and have a valid driver's license.
• Experience in hospice or long-term care preferred.
• Basic computer skills to document in EMR.

Responsibilities

• Perform personal care needs after assignment and attend to requests promptly.
• Provide patient and family with respectful communication and confidentiality.
• Ensure safety and use equipment properly (foot stools, side rails, O2).
• Give baths, back rubs, oral hygiene, shampoos and change bed linens.
• Assist in dressing and undressing patients as assigned.
• Assist in feeding the patient as assigned; avoid homemade foods.
• Observe skin condition to prevent tissue breakdown over bony areas.
• Report pain or changes to the RN Case Manager promptly.
• Assist in ambulation and prescribed exercises by the hospice nurse/therapist.
• Provide ROM and transfers/positioning as directed.
• Assist with self-administered medications as allowed by state regs.
• Assist with bedpans, urinals and light housekeeping tasks.
• Perform ROM and simple procedures with competency when ordered.
• Provide respite for patient/family/caregiver when onsite.
• Keep living area clean and orderly; follow procedures and conduct standards.
• Participate in QA/PI teams; be respectful of patient/family environment.
• As mandatory reporter, report health dept requirements within 14 days.
• Report suspected abandonment, abuse, or neglect per policy.
