Job summary

Affinity Hospice Holdings, LLC seeks a dedicated Certified Nursing Assistant (CNA) to support patients in their homes under RN supervision. You will observe, report, and document patient conditions while providing compassionate personal care and comfort to individuals and families.

Qualifications

• Must have empathy for ill, injured, frail individuals.
• CPR certification must be current.
• Possess and maintain a valid driver’s license and reliable vehicle.

Responsibilities

• Provide personal care and attend to patient requests promptly.
• Ensure safety and proper use of equipment (e.g., O2, rails).
• Assist with bathing, oral hygiene, dressing, feeding, and comfort measures.
• Report patient condition changes to case manager and document care.
• Adhere to documentation procedures and professional conduct.
