Your Health in the Little River area is seeking a dedicated Certified Nursing Assistant to service hospice patients at homes and ALFs. You will deliver medications, DME, and coordinate telehealth visits while traveling daily in your own vehicle. Full-time, 8-hour shifts, 8:00AM-5:00PM, Monday-Friday.

You’ll work with our physician group serving SC/GA, providing compassionate care, ADL support, and collaboration with care teams to ensure quality patient outcomes.

Qualifikationen

• Certified Nursing Aide certification valid and in good standing.
• Phlebotomy experience preferred; willingness to train if needed.
• Experience in public health or home health settings preferred.
• Experience with DME delivery or logistics preferred.
• Proficient with technology tools for remote monitoring, telemedicine and EMR.

Aufgaben

• Collaborate with care team to ensure coordinated patient care.
• Communicate effectively by phone, email, Teams and in person.
• Provide compassionate personal care for hospice patients at home.
• Assist with mobility, transfers and fall prevention.
• Perform light housekeeping related to patient care.
• Monitor patient condition changes and document them.
• Report significant changes to hospice nurse promptly.
• Support ADLs; promote dignity, comfort and independence.
• Provide emotional support to patients and families.
• Assist with meals and hydration per care plans.
• Deliver medications to patient homes and reconcile instructions.
• Coordinate telehealth visits with pharmacists and providers.
• Perform routine venipuncture for labs and label specimens.
