Your Health in Charleston area is seeking a dedicated Continuous CNA to service hospice patients in the region. You will work closely with the interdisciplinary team to stabilize symptoms, ensure comfort, dignity, and safety, and provide quality care within regulatory guidelines.

This full-time, salary-based position requires travel to patients’ homes and surrounding facilities. A reliable vehicle and clean driving record are essential as you will cover a designated service area with 12-hour

Qualifikationen

• Certified Nursing Assistant (CNA) in good standing is required.
• Phlebotomy experience preferred or willingness to train to competency.
• Public health/home health experience preferred.
• DME delivery or logistics experience preferred.
• Proficiency with technology tools for remote patient monitoring, telemedicine, and EMR.
• Strong organizational and time management skills; able to multitask.
• Excellent written and verbal communication skills.
• Basic computer knowledge.
• Ability to work independently with minimal supervision.
• Must travel between offices/facilities and drive.
• Clean driving record and insured vehicle.

Aufgaben

• Collaborate with hospice care team to ensure coordinated patient care and adherence to care plans.
• Communicate with team via phone, email, Teams, or in person.
• Provide compassionate personal care in patients' homes (bathing, grooming, hygiene, dressing, toileting).
• Assist with safe mobility, transfers, repositioning, and fall prevention.
• Perform light housekeeping related to patient care (changing linens, cleaning areas).
• Monitor and document patient condition changes (skin integrity, appetite, comfort, safety).
• Report significant changes to hospice nurse or clinical team promptly.
• Support ADLs while upholding dignity and independence.
• Provide emotional support and companionship to patients and families.
• Assist with meal prep, hydration per care plans.
• Deliver medications, review instructions, reconcile lists for accuracy.
• Coordinate telehealth visits with pharmacists, specialists, and providers.
• Perform routine venipuncture and proper specimen handling.
• Collect blood samples during home or facility visits as needed.
