Job summary

Your Health is hiring a dedicated Certified Nursing Assistant (CNA) to serve hospice patients in our South Bay Mount Pleasant area. The CNA will visit patients in homes and ALFs, deliver medications and DME, and coordinate with providers to ensure timely access to care.

Responsibilities include ADLs support, mobility assistance, medication reconciliation, and telehealth coordination with pharmacists and clinicians. Reliable transportation and a clean driving record are required for daily travel.

Qualifications

• Certified Nursing Aide required with active licensure in good standing.
• Phlebotomy experience preferred; willing to train if not.
• Public health or home health background preferred.
• Experience with DME delivery or logistics is a plus.
• Proficient with technology tools for remote patient monitoring, telemedicine, and EMR.

Responsibilities

• Provide compassionate personal care to hospice patients in homes and facilities.
• Assist with safe mobility, transfers, and fall-prevention tasks.
• Perform light housekeeping related to patient care.
