Your Health is seeking a dedicated Certified Nursing Assistant to service hospice patients in the Aiken, SC area. The CNA will visit patients at their homes and Assisted Living Facilities, delivering medications and DME, coordinating telehealth, and supporting care teams to ensure timely and compassionate service.

The role requires reliable transportation, full-time hours (8:00 AM–5:00 PM, Monday–Friday), and strong communication skills to liaise with nurses, pharmacists, and family members.

Qualifikationen

• Certified Nursing Assistant certification in good standing.
• Phlebotomy experience preferred; willing to train if needed.
• Public health or home health experience preferred.
• Experience with DME delivery or logistics preferred.
• Proficient with EMR and telemedicine tools.

Aufgaben

• Work closely with care team members to ensure coordinated patient care per care plan.
• Communicate effectively via phone, email, Teams, Athena, and in person.
• Provide compassionate personal care to hospice patients at home or ALFs.
• Assist with safe mobility, transfers, repositioning, and fall prevention.
• Perform light housekeeping tasks related to patient care.
• Monitor and document patient condition changes and report to hospice nurse promptly.
• Support patients with activities of daily living while promoting dignity and independence.
• Provide emotional support and companionship to patients and families.
• Assist with meal preparation, feeding, and hydration per plan of care.
• Deliver medications to homes, review instructions, and reconcile lists.
• Coordinate telehealth visits between patients and providers.
• Perform venipuncture for lab testing and proper specimen handling.
• Collect blood specimens during visits as needed.
