Your Health Organization in Myrtle Beach, SC, is hiring a Certified Nursing Assistant (CNA) to serve hospice patients in their homes and ALFs. The role involves direct patient care, medication delivery, DME handling, and coordinating telehealth visits as part of a dedicated care team.

Travel is required daily with reliable transportation; the position is full-time, 8-hour shifts (8:00 AM–5:00 PM, Monday–Friday). Strong communication and organizational skills are essential for success.

Qualifications

• Certified Nursing Assistant in good standing.
• Phlebotomy experience preferred.
• Home health experience preferred.

Responsibilities

• Provide compassionate personal care to hospice patients at home and ALFs.
• Assist with mobility, transfers, and fall prevention.
• Deliver medications and review instructions.
• Coordinate telehealth visits with pharmacists and specialists.
• Document patient condition and report changes.
• Ensure safe use of DME and manage supplies.
