Job summary

Interim HealthCare of the Midlands in Columbia, SC is seeking a compassionate Hospice Aide (CNA) to deliver patient-centered end-of-life care in patients' homes on a PRN basis. You will provide essential daily living support, monitor comfort, and collaborate with a caring team to honor patient dignity.

Applicants should be a licensed CNA in South Carolina with a valid driver’s license and reliable transportation.

Qualifications

• High school diploma or equivalent; active SC CNA.
• Valid driver's license, auto insurance, and reliable transportation.
• Compassionate, good communicator; ability to lift up to 50 lbs.
• Hospice experience preferred.

Responsibilities

• Care for seniors and patients in their homes.
• Assist with daily activities: bathing, toileting, dressing, grooming, hygiene.
• Assist with ambulation, transfers and range of motion exercises.
• Check vitals, document changes, and monitor glucose as needed.
• Collaborate with home health nurses to follow the care plan.
• Notify supervisor of issues requiring changes in care or equipment.
