Job summary

Bristol Hospice - Rogue Valley in Grants Pass, OR is seeking a dedicated Certified Home Health Aide (CHHA) to join our team. You will work under the supervision of a Registered Nurse to provide personal care and comfort to patients in their homes.

The role requires CNA certification (CA HHA if applicable), CPR, and completion of 75 hours of classroom and supervised training. Experience in hospice or medical settings is preferred; driving and background checks are part of the process.

Qualifications

• Must be a CNA in good standing.
• California: must have a current HHA certification in CA.
• Minimum 1 year experience in community health/hospice or medical/surgical settings preferred.
• Must have completed 75 hours of classroom and supervised practical training.
• Licensed driver with insured vehicle in good working order.
• Willingness to submit to background check annually.
• Current CPR certification.

Responsibilities

• Perform personal care tasks as assigned and report any issues to Case Manager.
• Provide patient and family with compassionate communication within ethical care boundaries.
• Ensure safety and proper use of equipment (foot stools, rails, O2, etc.).
• Provide personal care including baths, oral hygiene, and changing bed linens.
• Assist in dressing, meals planning, and feeding as required.
• Take and record vital signs when ordered and demonstrate competency.
• Monitor skin condition and prevent tissue breakdown.
• Assess patient condition and report changes to Case Manager.
