Job summary

Bristol Hospice is seeking a Certified Home Health Aide to join its interdisciplinary team, working under a Registered Nurse to support patients in their homes. The role emphasizes compassionate care, confidentiality, and adherence to the plan of care.

Qualified CNAs with CA HHA certification and CPR are preferred; driving may be required. The position offers full-time hours, with a patient-centered, team-based environment and opportunities for professional growth.

Qualifications

• Must be a Certified Nursing Assistant in good standing.
• California residents must have current Home Health Aide certification in good standing.
• Preferred 1 year in community health/hospice or medical/surgical environments.

Responsibilities

• Provide personal care: baths, grooming, oral hygiene, and changing bed linen.
• Assist with meals, feeding, and monitoring nutrition.
• Take and record vital signs as ordered.
• Ensure safety and proper use of equipment in the home.
• Assist with mobility, transfers, and range-of-motion activities.
• Support patient and family with communication and plan of care documentation.
• Maintain a clean, safe living environment for the patient.
• Participate in quality assessment and care planning.
