Job summary
Bristol Hospice is seeking experienced Certified Home Health Aides (CHHA) to support patients and families in home hospice settings. You will work under a Registered Nurse and collaborate with an Interdisciplinary Group Team to ensure compassionate, dignified care that meets the plan of care and confidentiality standards.
The ideal candidate will hold a CNA in good standing, California HHA certification if applicable, a valid driver's license, CPR, and prior hospice or community health
Qualifications
- Must be a Certified Nursing Assistant (CNA) in good standing.
- California regulation requires CNA in good standing and HHA certification in CA.
- Preferred 1 year of community health/hospice or medical/surgical experience.
- Licensed driver with insured vehicle and valid license.
- Willingness to undergo background check and annual checks.
- Current CPR certification.
Responsibilities
- Provide personal care including baths, grooming, and hygiene.
- Assist with meals, feeding, and nourishment planning.
- Take and record vital signs as ordered.
- Assess skin integrity and report changes to Case Manager.
- Assist with ambulation, transfers, and ROM as directed.
- Support with medications reminders within state regulations.
- Maintain clean, safe living areas and document care procedures.
- Provide respite support to family/caregiver as needed.
