Job summary

Bristol Hospice in Brookhaven, MS seeks an experienced Certified Home Health Aide (CHHA) to support patients in their homes under RN supervision. You will perform personal care, meals, and mobility assistance while ensuring confidentiality and comfort.

The ideal candidate holds CNA/HHA certification, CPR, and a valid driver’s license, with experience in hospice or community health being a plus. Flexibility and strong communication are essential in this role.

Qualifications

• Must be a CNA in good standing; California requires an HHA in good standing.
• Minimum 1 year experience in community health, hospice, or related field preferred.
• Valid driver's license and insured vehicle as required by organization/state.
• Ability to follow plan of care and documentation processes precisely.
• Current CPR certification and background check completion.

Responsibilities

• Provide personal care, bathing, grooming, and oral hygiene.
• Attend to patient requests promptly and report changes to Case Manager.
• Assist with meals, nutrition, and feeding as assigned.
• Assist with ambulation, transfers, and positioning.
• Monitor vital signs when ordered and record data.
• Maintain safe home environment and use equipment properly.
• Document care provided per organization procedures.
• Offer respite for family/caregiver as appropriate.
