Job summary

Bristol Hospice in Sonora, CA is seeking an experienced Certified Home Health Aide (CHHA) to join our Interdisciplinary Group Team. You will work under the supervision of a Registered Nurse and provide personal care to meet patients' needs and comfort.

The CHHA role supports families, adheres to California CNA/HHA requirements, and contributes to compassionate, dignified hospice care. We value integrity, respect, and teamwork, and offer training and growth opportunities.

Qualifications

• Must be a CNA in good standing with CA Home Health Aide certification.
• Preferred 1 year experience in community health/hospice or medical/surgical settings.
• Must have completed minimum 75 hours of classroom and supervised practical training.

Responsibilities

• Provide personal care such as baths, grooming, and oral hygiene.
• Assist in dressing, meals, and daily living activities for patients.
• Take and report vital signs and changes to the Case Manager.
• Support safe ambulation, transfers, and range of motion as directed.
• Maintain patient living area and adhere to care plan documentation.
