Job summary

Bristol Hospice - East Bay LLC in San Jose, CA is seeking an experienced Certified Home Health Aide (CHHA) to join our Interdisciplinary Group Team. You will assist with personal care under RN supervision, promote comfort, and support the patient and family in a compassionate hospice setting.

The role requires CA CNA and HHA certifications, CPR, and a valid driver's license. Prior hospice or home health exposure is preferred; competitive hourly pay and comprehensive benefits offered.

Qualifications

• Must be a Certified Nursing Assistant (CNA) in good standing.
• California residents must hold a current Home Health Aide (HHA) certification in good standing.
• Prefers 1 year experience in community health/hospice or medical/surgical environments.
• Minimum 75 hours classroom and supervised practical training.
• Licensed driver with insured vehicle in accordance with state requirements.

Responsibilities

• Perform personal care and attend to patient requests.
• Provide patient and family with respectful communication and confidentiality.
• Ensure safety and proper use of equipment.
• Assist with baths, hygiene, dressing, meals, and mobility.
• Take vital signs and document changes when ordered.
• Assist with medications reminders and self-administered meds as allowed.
• Maintain clean and orderly living area and plan of care documentation.
• Support caregiver and provide respite as needed.
• Participate in quality improvement activities.
