Job summary

Affinity Hospice is seeking a Certified Nursing Assistant for full-time, in-home patient care. The role involves direct patient assistance under RN supervision, documenting observations, and promoting comfort during visits to patients' homes.

Eligible candidates will have a high school diploma or equivalent, CPR certification, and a valid driver's license with vehicle insurance. Evenings, weekends, and holidays may be required. We offer PTO, benefits, and advancement opportunities.

Qualifications

• High school diploma or equivalent is required.
• Current CPR certification must be maintained.
• Licensed driver with insured vehicle per organization requirements.

Responsibilities

• Perform personal care tasks including baths, back rubs, oral hygiene, shampoos, and changing bed linen as often as assigned.
• Assist in dressing and undressing patients as assigned.
• Assist in feeding the patient as assigned.
• Assess and report on patients’ condition and significant changes to the case manager.
• Adhere to documentation and care procedures and standards of personal and professional conduct.
