Job summary

Centric Home Health and Hospice is seeking a compassionate Certified Nurse Aide (CNA) to provide personal care and support services to patients in their homes. You will work under the supervision of a Registered Nurse to assist with daily living activities and maintain a safe home environment.

The role requires Texas CNA certification, CPR, and reliable transportation. Prior hospice or home health aide experience is preferred, with a flexible schedule and ability to travel locally.

Qualifications

• Must be certified as a Nurse Aide in Texas
• CPR certification required
• High school diploma preferred
• Reliable transportation and valid driver's license required
• Experience in hospice or home health Aide preferred

Responsibilities

• Assist patients with activities of daily living
• Support safe transfers and ambulation
• Maintain a clean and safe home environment
• Report changes to supervising RN

• Operate durable medical equipment as needed
