Job summary

Centric Home Health and Hospice is seeking a compassionate CNA to provide in-home personal care under RN supervision. You will assist with daily living activities, ensure patient safety, and support families while maintaining accurate care documentation.

Requirements include a TX CNA certification, CPR, and reliable transportation. Hospice or nursing assistant experience is preferred, with a flexible schedule for travel to patient homes.

Qualifications

• High school diploma preferred.
• Completed approved Nurse Aide training and competency program.
• Current TX Nurse Aide certification.
• Current CPR certification.
• Reliable transportation, valid driver’s license, and current auto insurance.
• At least one year of Hospice Aide or Nursing Assistant experience preferred.

Responsibilities

• Assist patients with activities of daily living including bathing, grooming, nutrition and mobility.
• Support safe transfers and ambulation.
• Maintain a clean, safe, and comfortable home environment.
• Report changes in patient condition to supervising RN.
• Maintain accurate documentation of care.
• Follow infection control and safety procedures.
• Communicate respectfully with patients, families, and staff.
• Use and handle durable medical equipment as needed.
