Job summary

Harmony Home Health & Hospice is seeking a compassionate Hospice Aide (CNA) to join our care team. You will provide personal care, support clients and families, and help maintain comfort and dignity in a hospice setting.

Under the supervision of a nurse, you will document vital signs, communicate changes, and collaborate with diverse colleagues. Valid CNA certification, CPR/BLS, and a valid driver’s license with insurance are required.

Qualifications

• Current State CNA Certification in good standing.
• Current CPR with BLS card.
• Valid driver's license with minimum auto insurance and an operational vehicle.
• Knowledge of Federal, State, and local laws governing hospice operations.
• Strong organization skills and ability to work without direct supervision.
• Calm in emergencies or crisis, around clients who may be dying.
• Commitment to professional boundaries.
• Experience documenting on an EMR system.

Responsibilities

• Perform client personal care needs and support other daily needs per the hospice plan of care.
• Record and report vital signs and health information per the plan of care.
• Communicate effectively with a diverse client population and co-workers.
• Work under nurse supervision; promptly report concerns, consult with a nurse on treatment changes, and submit documentation timely.
