Job summary

Seasons Hospice seeks a compassionate CNA to deliver personalized care in patients' homes or facility settings. You will assist with daily living activities, monitor health changes, and provide emotional support as part of a caring team.

Requirements include a CNA program or HHA cert and at least 1 year of direct care experience preferred. Expect physical tasks, lifting, and flexible scheduling in a meaningful hospice environment.

Qualifications

• High school diploma or equivalent preferred.
• Completion of an approved CNA program or HHA certification course.
• At least 1 year of direct care experience preferred, hospice/home care a plus.
• Ability to lift up to 75-100 pounds and stand or walk for long periods.
• Must pass pre-employment screenings and background checks.

Responsibilities

• Assist with bathing, grooming, dressing, toileting and personal hygiene.
• Change linens, wash laundry, clean living areas and maintain a safe environment.
• Measure and report vital signs and observe health changes.
• Assist with feeding, hydration, ambulation and patient positioning.
• Document care and communicate with the nursing team.
• Provide companionship and emotional support.
• Accompany patients to appointments and assist with mobility.
• Support medication adherence as directed by nurses or physicians.
