Job summary

Qualifications

• Education: High School Diploma preferred; approved aide programs.
• Licensure: Must have a current driver’s license.
• Experience: One year as Hospice Aide or Nursing Assistant preferred.
• Training: Completion of an approved aide competency program.

Responsibilities

• Provides direct patient care to patient under RN supervision.
• Assists with safe transfers and ambulation per policy.
• Reports changes and documents pertinent information to ensure continuity of care.
• Practices infection control and maintains a clean home environment.
• Supports patient nutrition and fluid intake.
• Communicates effectively with patient, family and hospice staff.
