Job summary

Patriot Healthcare in Conway, South Carolina is seeking a Hospice Certified Nursing Assistant to provide personal care to assigned patients in line with nursing assessments and the care plan, under the supervision of registered nurses.

You will participate with the Interdisciplinary Group in developing and implementing the patient/family plan of care, assist with daily activities, and document tasks while ensuring comfort and dignity.

Responsibilities

• Provide personal care services to assigned patients per nursing assessments and care plan.
• Assist with daily living activities and comfort measures for hospice patients.
• Document tasks performed and report to supervising nurse.
• Collaborate with IDG to develop and implement the patient/family plan of care.
