Job summary

JOL Healthcare, Inc. is seeking a Hospice Certified Nursing Assistant to provide personal healthcare and related services to patients in their homes.

Under the supervision of an RN, you will perform direct care, assist with transfers and ambulation, and document observations for the care plan. You will adhere to infection control, maintain a safe home environment, and communicate with patients, families, and hospice staff.

Qualifications

• Must be able to read, write, and verbally report clinical information to patients, representatives, caregivers, and hospice staff.
• Demonstrates interest in the welfare of the ill and elderly.

Responsibilities

• Provides direct patient care under the direction of the RN and according to the Aide Plan of Care.
• Assists patients with safe transfers and ambulation per Hospice policy.
• Reports changes in patient condition and documents pertinent information and care provided.
• Practices infection control principles and maintains a clean, safe, and comfortable environment.
• Promotes respectful communication with patients, families, and staff.
• Assists with proper nutrition and adequate fluid intake.
• Works in patients’ homes under various conditions and travels locally as needed.
