Job summary

Qualifications

• Ability to read, write and report clinical information clearly.

Responsibilities

• Provide direct patient care under RN supervision in patients' homes.
• Assist with transfers, ambulation and daily activities.
• Document care provided and report changes to RN.
• Maintain infection control and safe home environment.
• Communicate with patients, families and hospice staff respectfully.
• Support nutrition and hydration needs and comfort.
• Assist with procedures per Hospice policy.
